First Visit

Your doctor has referred you to our practice for a specialist opinion about a urological condition. Please arrive 15 minutes before your appointment and make yourself known to the receptionist.

For your first visit, you will need to bring the following items:

You will receive an information sheet and questionnaires that assist in the assessment of your problem through e-mail. Please complete them at home and bring them with you to your appointment. If you do not have an e-mail address, these can be posted to you, or you can fill them out when you arrive. You will also be given written information pertaining to the collection, use and disclosure of your personal information and financial obligations prior to your appointment.

We allocate 20 minutes for an initial consultation, 10 minutes for a follow-up consultation and try our best to keep to schedule. We understand that your circumstances may change and request that you contact us immediately if you are running late or cannot attend your appointment. Failure to attend the appointment without prior notice may result in a charge.

Privacy Matters

As a condition of employment, all staff members are bound by strict confidentiality regulations regarding medical records. The contents of your medical file will not be released without your consent.
